Unexplained Implantation Failure: Which Tests Help?

A large share of RIF stays “unexplained” after a full, guideline-based work-up — and that is a finding, not a failure of your doctor. The real risk is over-testing: immune panels, “natural killer” cell assays, and exotic treatments are widely marketed but are not endorsed by ASRM or ESHRE for routine RIF. This article separates what the evidence supports from what it doesn’t, so you can say yes to useful tests and no to speculative ones. General education, not individual medical advice.

What the evidence supports

  • Cavity evaluation (SHG/hysteroscopy) and treatment of found lesions.
  • PGT-A in selected cases (recurrent aneuploidy, advanced age).
  • Thrombophilia testing only with a personal/family history or antiphospholipid syndrome.
  • Treating uterine abnormalities and lifestyle factors (BMI, smoking).

What is NOT routinely recommended

Test / treatmentWhy it’s contested
Routine immune assays (NK cells, alloimmunity)No validated diagnostic threshold; not guideline-recommended
Immunotherapy (IVIG, intralipid, steroids) without indicationNo consistent benefit; potential harms
Endometrial scratchMajor RCTs (e.g., SSF trial) show no benefit, possible harm
Assisted hatchingNo consistent improvement in RIF
“Receptivity” add-ons without a clear planUseful only in defined protocols; easy to over-order

The over-testing trap

Clinics differ. A test being available is not the same as indicated. Ask for the evidence and the expected change in management for any test offered.

What “unexplained” can and cannot mean

  • Can mean: you’ve completed a reasonable work-up and the next step is a careful, evidence-based retry or a documented discussion of donor/gestational-carrier options.
  • Cannot mean: that a hidden, expensive panel will inevitably reveal the cause, or that unproven treatment is warranted.
