How to Read an IVF Embryology Report When Embryos Stop Growing
When embryos stop growing, the embryology report is your most concrete source of truth — but it is written in shorthand. This guide explains the usual fields so you can understand what happened in your cycle, ask better questions, and avoid misreading a normal variation as a failure. The two numbers that matter most are your fertilization rate and your blastulation rate; the day-3 grade describes competence but does not diagnose cause. The fields, decoded

A worked example
Report says: 10 eggs, 8 MII, 6 fertilized (2PN), day-3 embryos graded 8-cell/<10% frag (×3), 7-cell/20% (×2), 6-cell/30% (×1); all arrested by day 4; 0 blastocysts.
What this tells you:
- Fertilization rate = 6/8 = 75% → normal. Sperm–egg interaction was fine.
- Arrest at day 4, no blastocyst → the issue is post-fertilization developmental competence, not fertilization.
- With 6 good-quality day-3 embryos yet zero blastocysts, the leading suspects are embryo chromosome abnormality / oocyte competence, not the lab.
Contrast: if only 1 of 8 eggs fertilized (low 2PN), the story is about fertilization, and ICSI or a sperm work-up belongs in the conversation.
Glossary you can keep
- Cleavage-stage embryo — an embryo at day 1–3, dividing by splitting (not growing in size).
- Compaction — day 3–4, cells fuse; a common arrest checkpoint.
- Blastocyst — day 5–6 cavity stage; required for highest pregnancy potential.
- Fragmentation — cellular debris; higher % = lower competence, but not a diagnosis alone.
What the report can and cannot tell you
- Can show: fertilization efficiency, day-3 quality, the exact arrest day, and whether any embryo reached blastocyst.
- Cannot show: why an embryo was aneuploid, whether sperm DNA fragmentation contributed, or your egg “quality” as a fixed label. Those need the pre-next-cycle review and sometimes sperm DNA testing (article).
